Mr. Singh’s Journey Home

Speaking no English, Mr. Singh traveled from India to the United States to work. Two months later Mr. Singh suffered a stroke and was admitted into a US hospital where he received care for the next 18 months.  He was unable to coordinate his transition home and into appropriate care.  Following an 8-step process, the family, healthcare facility, and medical providers (home and abroad) were able plan his return.  His family was there to great him upon arrival and down on their knees with gratitude for those who helped him.
