We help healthcare organizations manage patients safely back to their homelands.  We provide transition services for health-challenged individuals who are unable to navigate their way home from U.S. health care systems.  We assist health care systems with transfers of patients that are non U.S. Citizens and in need of transport to their homeland, and to family members or foreign healthcare facilities that will care for them.  

We have experience with preparing, arranging for and transporting citizens back to their country of origin.  We understand the complexities involved in discharging a patient to a foreign family or hospital, and our entire staff is dedicated to simplifying their process and making it comfortable to both the patient and their family members.  

We develop the safest and most cost-effective solution to deliver the patient to the required destination. We have experience working with global cultures and diversity, foreign consulates, arranging and preparing citizenship paperwork and resolving customs and clearance issues.  Our staff consists of registered nurses and rehabilitation personnel, and field experts on international cultures, home assessments, security and transport.

Many undocumented immigrants have long hospital and nursing home stays when they cannot be safely discharged and don’t have funding for another level of care.  Keeping patients longer than necessary is an expensive proposition for the hospital, as well as exposing patients to potential infections and other risk factors associated with a hospital stay.  

Worldwide Transitions, Inc. reduces the cost of unreimbursed medical expenses incurred by hospitals, sub-acute care facilities and private practitioners who are paying out-of-pocket for costly services in caring for undocumented immigrants.

We help the patient travel back to their home safely, and make arrangements for their well-being after they arrive home.  It is a way for patients to get the care they need in their homeland after discharge, rather than keeping them in an acute care facility they no longer need, or in sub-acute care that could be provided for by their own families.
